2K and Hangar 13’s action-adventure game, Mafia: The Old Country, is now available, and fans have been hard at work discovering the game’s secrets and Easter eggs. We’ve put together some of the standout ones you may have missed during your playthrough.
Whether it’s legacy cameo appearances that tie the series together, secret NPC interactions, collectible items, or real world references, Mafia: The Old Country features a worthwhile selection of Easter eggs for fans to uncover. Read ahead to learn about the ones that stood out to us.

Cameos
Fans of the Mafia series will be thrilled to discover younger versions of iconic characters from previous entries. These include Leo Galante, Frank Vinci, Giuseppe Palminteri, and Sam Trapani, each appearing in subtle but meaningful roles long before their rise to prominence in Mafia and Mafia 2.
Collectibles
Exploration in Mafia: The Old Country is rewarded with a variety of collectibles, including handwritten notes, vintage newspapers, saints’ cards, photographs, and Trinacrias. Many of these items offer small slices of world-building, a letter hinting at political unrest, or a shrine tucked away in a quiet alley, that expand the narrative beyond the main missions. One standout collectible is an arrest record for Mafia 1’s central antagonist, Don Salieri, found in the Guardia HQ during Chapter 6.
Real World Reference
Holy cards dedicated to Sicilian saints like Santa Rosalia appear in several locations in Mafia: The Old Country, giving cultural authenticity to its setting and story. The game’s Chapter 12 takes its title from the opera La Forza del Destino, a program that is performed during its story events. The opera features a moment where a father disowns his daughter because of her relationship with a man, mirroring the secret romance between protagonist Enzo and Don Torrisi’s daughter Isabella. In reality, La Forza del Destino is infamous for its so-called curse, linked to a series of misfortunes surrounding its production.
Hidden NPC Interactions
Mafia: The Old Country features optional conversations with NPC characters that are easy to miss. It’s therefore recommended to exhaust all possible dialogues with characters like Isabella and Cesare in order to gain extra insight into the game’s narrative.
Random Discovery
YouTuber Manugames92 discovered a gramophone sitting on a rocking chair hidden in the ruins section of the game. It’s tough to say what it’s meant to imply, but perhaps it’s some sort of a reference to the game of musical chairs.
Mafia: The Old Country is now available for PS5, Xbox Series X|S, and PC.
