Pathologic 3 Hotfix 6 delivers a wide-ranging set of fixes aimed at quest progression, character states, and interaction logic across multiple in-game days. Several long-standing issues involving Eva, the water quest, and repeat run blockers have been resolved to improve overall playability.
The hotfix also includes numerous targeted adjustments to dialogue triggers, inventory behavior, map markers, and interaction colliders. Together, these changes reduce soft locks and unintended quest failures while smoothing moment-to-moment interactions.

Quest Fixes
- Fixed Eva’s incorrect state on Days 11 and 12, preventing her from standing in unintended locations after specific actions.
- Fixed Eva disappearing on Day 8, which could block saving the bull.
- To restore Eva in an already broken save, the related quest must be reset twice.
- Restored disappearing barrels on Day 4 and moved associated map markers.
- The water quest can now be completed without issues.
- Fixed variable resets when examining Peter on Day 8, which could block quest progress on repeat runs.
- Added missing dialogue for Victor after reconciliation on Day 2.
- Fixed a hospital investigation mindmap node on Day 5.
- Fixed resetting patient quests on Day 7.

- Players who already completed the quest must replay it to unlock the achievement.
Character and World Logic
- Georgiy no longer discusses the Polyhedron on Day 8 if he is already dead.
- Fixed conditions on the People tab for the Changeling, Block, Karminsky, and Lara.
- Patient Patchwork no longer escapes the hospital ahead of schedule.
- Unlocked the door to Emilia’s house on Day 11.
- Fixed the door in the Thanatica during a flashback conversation with the Inquisitor.
- Removed a map marker that could become stuck after taking power on Day 3.
- Moved Yulia slightly near the Town Hall on Day 3 to improve visibility.
- Moved Capella on Day 4 to prevent overlapping quest usage.
UI and Interaction Improvements
- Fixed marker text while searching for Nina’s monument.
- Increased interaction colliders for reading tags when examining Saburova on Day 9 and The Good on Day 10.
- Fixed a mindmap node and marker behavior tied to multiple investigations.
- Items now return correctly if the player leaves the room early during Saburova’s operation.
- Items in the scary house on Day 10 now display the correct icons.
Inventory and Items
- Twyre now correctly disappears from the inventory when used.
- Fixed several crafting recipes.
- Fixed inventory behavior related to quest item removal.
Environmental and Technical Fixes
- Adjusted issues that could occur when switching weather and time of day.
- Added a butterfly to a gravestone on Day 6, tied to gameplay logic rather than a cosmetic change.
